ISLAMABAD: The Federal Board of Revenue (FBR) has decided to intensify action against private-practice doctors, medical clinics, and private hospitals allegedly involved in large-scale tax evasion amounting to billions of rupees.
Furthermore, the decision follows a detailed data analysis conducted by the FBR, which has revealed widespread non-compliance within one of the country’s highest-earning professional sectors.
In this sense, as per the official data shows that there are 130,243 registered doctors across Pakistan.
However, during the current tax year, only 56,287 registered doctors filed income tax returns. More than 73,000 registered medical professionals did not submit any income tax return, despite being engaged in private practice and other income-generating activities.
In this regard, FBR officials noted that these doctors are part of professions generally considered among the highest income brackets.
The data mentions a stark mismatch between the visible volume of private medical practice and the income declared to tax authorities. So far, 31,870 doctors declared zero income from private practice in 2025, while 307 doctors reported losses.
This stands in contrast to the situation in major cities, where private clinics reportedly remain busy with patients throughout the day. Meanwhile, only 24,137 doctors declared any business income, even among those who filed tax returns.
In addition, 17,442 doctors reported annual incomes exceeding Rs1 million but paid an average daily tax of just Rs1,894. Similarly, 3,312 doctors earning between Rs5 million and Rs10 million annually paid an average daily tax of Rs1,594, and the top 3,327 doctors with incomes exceeding Rs10 million paid around Rs5,500 per day.
Additionally, 38,761 doctors declared annual incomes below Rs1 million, paying an average daily tax of Rs791.
FBR officials noted that the situation contrasts sharply with salaried government employees in grades 17 and 18, who often pay more tax monthly than many doctors pay over an entire quarter, largely because salaried income cannot be concealed.
The FBR stated that restoring fairness in the tax system requires urgent and effective enforcement measures.
